Job Description
The Behavioral Care Coordinator - LCSW reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Behavioral Care Coordinator - LCSW, under the supervision of a staff psychiatrist, provides evidence-based care in the diagnosis and treatment of mental illness in adults, and geriatric patients (as qualified) in the outpatient setting.  This position provides individual, group, and family psychotherapy, as well as crisis intervention services.  The Behavioral Care Coordinator - LCSW ensures that the quality of care is safe, effective, patient centered, timely, efficient, and equitable.    
 
Responsibilities:
Implements and adheres to the principles of the Collaborative Care model of an Integrated Behavioral Health Program into a Primary Care setting.
Conducts and monitors population screenings and provides appropriate follow up care as needed. 
Renders an evidence-based opinion regarding the patient’s mental status, competency, prospective dangerousness, diagnosis, and need for psychiatric treatment.
Works collaboratively with all staff on the treatment team to ensure seamless psychiatric care.
Performs evidence-based psychotherapy and proactively reviews treatment plan with supervising psychiatrist.  Will recommend treatment modifications as needed.
Educates patients and primary care providers and staff on the principles and benefits of Collaborative Care
Support primary care providers as needed with real-time formal and informal health consultations and by keeping them informed of patient outcomes and progress
Determine the appropriateness of each patient referred to Collaborative Care arranges alternatives for patients deemed inappropriate.  Also arranges outside resources as needed for enrolled/discharged patients
Updates and maintains patient care registry
Provides crisis intervention to patients in clinic as needed
