RN Supervisor Concierge Nursing Services

Meet clients and families in their home setting 
Develop a plan of care including nursing and support services that meet the client's needs
Determine what care giver team members are appropriate to meet the client's needs and manage that team ongoing
Conduct comprehensive assessments of clients' health status, including physical, emotional, and psychosocial factors, to ensure holistic care planning.
Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate seamless delivery of care and optimize client outcomes.
Monitor and evaluate clients' responses to interventions and adjust care plans accordingly to promote optimal health and well-being.
Provide education and support to clients and their families on disease management, medication administration, and lifestyle modifications to empower them in self-care and promote long-term health goals.
Available to client and team on a daily basis to ensure services are provided timely and in full compliance with the clients stated desires.
Requirements
COVID vaccination required
Licensed Registered Nurse. BSN preferred
Supervisory experience
Prior home care experience REQUIRED
Private pay experience preferred
3+ years clinical experience
Valid US Drivers License
